Introduction 

Haemorrhoids: 

Piles (haemorrhoids) are venous swellings in the tissues around the anus. Those above the dentate line (the point where modified skin of the outer anal canal becomes gut epithelium) which usually protrude into the anal canal are termed internal haemorrhoids while those below this point are called external haemorrhoids. 

One study in India showed that approximately 80% of the cases are in the 21-50 year age group. The percentage of males and females was 67% and 33% respectively. Due to internal pressure, internal haemorrhoids tend to congest, bleed and eventually prolapse, while in external haemorrhoids painful thrombosis may develop. The study further suggests that constipation, spicy food and straining are the main causes in 62% of cases. Alcohol, emotional stress or sedentary habits have also been reported to be predisposing factors for haemorrhoids. 

Varicose veins: 

This condition is most often seen in the leg veins, which become dilated and appear slightly twisted or tortuous. This could be due to weakness of connective tissues, valve malfunction or inadequate venous return. Predisposing factors are pregnancy, adiposity, malignancy, prolonged standing and immobilisation. If not treated, phlebitis and thrombosis may complicate varicosis. In cases of chronic venous insufficiency congestion of skin and oedema, pigmentation, formation of scaly eruptions (eczema-like) and ulceration (ulcus cruris) may occur.

Proven indications of the individual ingredients in the sphere of claimed action: 

Aesculus hippocastanum acts on veins and lower bowel, producing engorged haemorrhoidal veins. It acts on blind or bleeding haemorrhoids with sharp shooting pains up to the back, sore raw anus, feeling of fullness and prolapse. Veins are distended in general. 

Arnica montana affects the venous system, inducing stasis and haemorrhage. It is effective in the treatment of varicose veins and haemorrhoids, tenesmus, wound healing of ulcus cruris, and injuries due to trauma or over-exertion. 

Calcarea fluorica is a powerful tissue remedy for varicose and enlarged veins. It acts on fissures and itching of the anus, bleeding or blind haemorrhoids, and weakness of venous walls and connective tissues. 

Carduus marianus has an affinity for the vascular system, especially veins and the portal system. It is indicated for varicose veins and ulcers, bleeding piles, burning pain, prolapse of the rectum and hard stool. 

Collinsonia canadensis acts on pelvic and portal congestion, haemorrhoids and constipation, vascular engorgement. Often constipation and diarrhoea occur alternately and painful piles with itching and pricking in the anus are present.
